Camp Program Registration Form 2026

(Must be completed prior to first day of camp)

Lunches and Snacks

Campers are expected to bring their own lunches and snacks.

Please note that we are a peanut-free environment to ensure the safety of all children.

Meal Schedule

• Breakfast will be offered between 7:30 A.M. and 9:00 A.M.
• An afternoon snack will be offered between 4:00 P.M. and 5:30 P.M. (Breakfast and an afternoon snack will be provided daily by Empowering Pathways Services).

Payment Information

Registration Fee (non-refundable):

• $50 per camper (one time fee) Covers administration costs, supplies, and holds your camper's spot in camp.

Camp Fee

• Weekly Camp Fee: $275 per camper
• Daily Drop- in Fee: $60 per day (if space allows)

Extended Care (Optional)

• Before Care (7:30 AM — 9:00 AM): $30 per week per child
• After Care (4:00 PM — 5:30 PM): $30 per week per child
• Before Care and After Care (Full Week): $55 per child
• Daily rate for before or after care: $20

Payment Methods - Empowering Pathways Services Camp

Payment is required to secure your child's spot in the camp. We accept the following payment methods

• Please include your child's first and last name and the specific camp dates in the payment notes

Consent and Authorization

Please read and sign below:

• I give permission for my child to participate in all summer camp activities organized by Empowering Pathways Services.
• I understand that the camp will take all reasonable precautions to ensure my child's
• I authorize the camp staff to administer basic first aid and seek emergency medical care if
• I confirm that all medical information provided is accurate and that I will supply any required medication along with written instructions.
• I understand that the registration fee is non-refundable, unless the camp is cancelled by Empowering Pathways Services.
• I will not hold Empowering Pathways Services or its staff liable for any accident, injury, or loss of property during camp activities.
